RCA Solutions is a company founded by healthcare professionals for healthcare
professionals. Our objective is to assist nurses, physicians, risk managers,
compliance officers, patient safety officers and general medical personnel to
reduce medical errors and save lives. To this end, we offer on-site consulting,
regional workshops and web training in the areas of Root Cause Analysis (RCA)
for Healthcare, Failure Mode and Effects Analysis for Healthcare (FMEA),
Facilitation Techniques and Healthcare Corporate Compliance.
Our team of educators have over forty years of combined experience working as
physicians, nurses, attorneys, engineers and risk managers. Working in
collaboration with one another the RCA Solutions team has produced educational
materials used to teach healthcare professionals how to conduct a thorough and
credible Root Cause Analysis (RCA) and Failure Mode and Effects Analysis (FMEA)
utilizing a three meeting approach and an interdisciplinary team. Educational
materials used for both on-site consulting and quarterly workshops include
training on how to effectively facilitate interdisciplinary meetings, the
meaning of systems errors and the importance of just culture in healthcare.
Presentations on other topics such as Corporate Compliance, Peer Review, Apology
Laws and Discovery Laws are available upon request.
