Denise G. Osborn, JD, MPH (2008)
Denise provides consultative services to hospitals, and medical device
manufacturers relating to risk management, corporate compliance, federal and
state legislation, root cause analysis (RCA) and failure mode and effects
analysis (FMEA). She has over fifteen years of experience as a litigator and
college instructor. She has worked on Capitol Hill and for a physician’s
association in Washington DC as a health policy specialist. Denise has a special
interest in issues pertaining to medical malpractice and patient safety. In
addition to a law degree and a masters in public health Denise also holds a
certificate in healthcare corporate compliance. Denise is the founder and owner
of RCA Solutions.

Mary Jane Fitzgerald, RN, JD
Mary Jane has worked in nursing and the medical profession since 1976. She has
extensive experience in promulgating policies and procedures in preparation for
JCAHO, Department of Health and DEA inspections. Over the past ten years she has
gained extensive experience in the area of claims management and has been
actively involved in the development of corporate level risk management policies
and procedures. Her presentations cover legal issues pertaining to patient
safety, risk, compliance and malpractice litigation. Mary Jane is originally
from New York and resides in Florida.

Susan Lesch, RN, BSN
Susan is a registered nurse with over 20 years critical care staff nursing as
well as management experience. She is currently the Director of a fifteen bed
ICU in a JACHO accredited community hospital. She works collaboratively with the
patient safety officer and risk management on process improvement including
tracking, trending and investigating incident reports. She is also instrumental
in creating clinical recommendations and implementing corrective actions. Susan
works with the management team and physicians identifying clinical issues
relative to patient safety and infection control and creates processes and
procedures ensuring the best clinical outcomes for the critically ill ICU
patient.

Carlos M. Garcia, BSAE, MBA, CM
Expert in process and systems improvement, design of performance measures, and
compliance with the Joint Commission healthcare accreditation requirements.
Experienced with various risk management tools including Root Cause Analysis
(RCA) and Healthcare Failure Mode and Effect Analysis (HFMEA). Six-Sigma coach
and project manager. Experienced in the application of IHI’s Advanced Clinical
Access methodology and queuing theory to patient throughput problems. Member of
committees and collaborative charged with helping healthcare organizations to
satisfy rigorous patient care quality measures from the VHA Deputy
Undersecretary for Healthcare Management, Washington, D.C. Holder of degrees in
engineering and business. Member of adjunct faculty at the University of Phoenix
and National University in San Diego, California, where he resides. Frequent
presenter and facilitator at the Western ACA-FIX Collaborative conferences and
certified Baldrige Examiner through the California Awards for Performance
Excellence program (CAPE). Seventeen years with major weapons manufacturing
contractors for the US Navy and Air Force, and eight years in the Veterans
Administration’s healthcare system. Carlos is a Six Sigma Black Belt candidate
through American Society for Quality (ASQ).

Kenneth Hirsch, PhD, MD
Dr. Hirsch is certified in Psychiatry and Addiction Medicine. Dr. Hirsch earned
his Ph.D in Clinical Psychology from the University of Toledo in Ohio and his
M.D. from George Washington University in Washington DC. He also completed both
internships and residencies with Galesburg Mental Health Center and Tripler Army
Medical Center. Dr. Hirsch has published articles ranging from mathematical
modeling through clinical treatments. Dr. Hirsch co-authored a book published
through HCPro on Root Cause Analysis and Patient Safety and co-created a
software product with Denise Osborn which won the coveted TETHIE award for
Patient Safety. Dr. Hirsch was the recipient of the Annual Award in Bioethics
from the Academy of Medicine in Washington, D.C., the Leo H. Criep Research Fund
Manuscript Award and the American Academy of Family Practice Dedication to
Teaching Award. Dr. Hirsch practices at the VA Hospital in Honolulu, Hawaii and
has worked collaboratively with Denise Osborn for over seven years.
