Root Cause Analysis is a valuable tool that can assist you to build effective
teams and improve communication among staff. Organizations can improve their
operations and the care they provide through probing and addressing the multiple
contributory factors and root causes that lead to adverse events and near
misses. The RCA analysis is retroactive in nature
and usually occurs subsequent to the occurrence of an event. In contrast,
Failure Mode and Effects Analysis is used proactively to improve policies,
procedures and protocols and does not necessarily require the presence of an
adverse event or near miss.
Whether your team is conducting a RCA or a FMEA, a three meeting process
